OPTION A - Licensed Churches, Temples, or Community Facilities The Health Department has provided guidelines specific to their location, please consult that facility for permitting requirements. Ask to speak with the “person in charge” of food events. They will be able to assist you further.
OPTION B For all other Temporary Food Events (i.e., an outdoor event or an event being held in a non-specific space), please submit an "Application for Temporary Food Service Establishment" to the Health Department. Click the link above to download the form.
